The Common Form

Introduction

To support the Long Term Conditions Management Programme, The Common Form has been developed by bestpractice in association with Midlands Health Network.

This module combines features from the Diabetes and CVD Management modules in a new design that emphasises speed and ease of use. It is a standards-based tool to assist clinical review, disease monitoring and clinical management.

Features

The Common Form has the following features:

  • Dynamic interface - sections of the form are hidden and shown automatically as data is entered into the form. This reduces clutter by only showing the appropriate parts of the form.
  • Real-time update - ‘risk alerts’ and management advice are updated in real-time as changes are made to the data. This streamlines the process by removing the need to click through to another screen for results.
  • Data pre-population - all relevant data from the MedTech patient record is pre-populated in the form. This means minimal typing and maximum speed.
  • PMS write-backs - where possible all information will write back into MedTech, e.g. smoking status, smoking cessation brief advice or refused smoking cessation will write back to the appropriate classification or screening information in MedTech. Manually entered lab results data is not verified, and will not write back and will only save on a completed form as an outbox document.
  • Patient recalls - the module can recall the patient for their next review or monitoring appointment.
  • Standards-based - the algorithms and rules behind The Common Form are written to reflect the latest NZGG Primary Care Handbook for CVRA, diabetes screening and management.
  • Fast CVRA - the CVRA is designed to collect only the minimum information in order to complete the risk algorithm, therefore avoiding unnecessary clicks and data entry.

Clinical Details

The Common Form main page consists of the Clinical Details section and the Clinical Management Advice section (shown below). The clinical details section is a form that permits the entry and editing of clinical data. It also contains several colour-coded risk alerts that are updated in real-time.
Clinical Management Advice

The clinical management advice section presents advice based on the data entered in the clinical details section. These advice lines are updated in real-time as data is entered or edited.
CVD Risk Classifications

The CVR Classifications screen is displayed when the icon on the CVD Risk Factor row of the Clinical Details section is clicked. This screen permits the writing back of classifications to MedTech.
Foot Check

The Foot Check screen is displayed when the icon on the Foot Check row of the Clinical Details section is clicked. This screen permits the entry of foot condition data for the patient.
Retinopathy
